INTRODUCTORY PERIOD The first 90 days of employment with BL Companies is an Introductory Period. During this time, Paid Time Off (PTO) will accumulate and may be used by you upon completion of the introductory period. You will be eligible for Holiday Pay and Company Paid Time Off after you have completed 30 days of employment. PAID TIME OFF (PTO) BL Companies provides employees with paid time off as well as paid holidays. The following is the PTO schedule, which accrues, based on a 40-hour work week: • Less than 5 years of relevant experience = 15 days
Eligibility Eligible employees must meet the following criteria:
• Have been employed with the company for at least 12 months. • Be a full time employee (part-time and temporary employees and interns are not eligible for this benefit). In addition, employees must meet one of the following criteria: • Have given birth to a child. • Be a spouse or committed partner of a woman who has given birth to a child. • Have adopted a child. The adoption of a child as a result of marriage is excluded from this policy.
• 5–10 years of relevant experience = 20 days • 10+ years of relevant experience = 25 days
Part-time employees scheduled to work at least 20 hours per week on a regular basis will be able to accrue PTO hours on a prorated basis.
PARENTAL LEAVE BL Companies provides up to two weeks of paid parental leave following the birth or adoption of an employee’s child. The purpose of paid parental leave is to enable the employee to care for and bond with a newborn or a newly adopted child. This policy runs concurrently with Family and Medical Leave Act (FMLA) leave, as applicable.
Company paid time off is provided from Christmas through New Year’s Day!

MEDICAL PLANS OVERVIEW We offer the choice of three (3) medical plans through Cigna. All of the medical options include coverage for prescription drugs, dental and vision coverage. To select the plan that best suits your family, you should consider the key differences between the plans, the cost of coverage (including payroll deductions), and how the plan covers services throughout the year. Health Insurance Our health insurance plan is a Preferred Provider Network (PPO) administered by Cigna Open Access Plus. There is a waiting period of 30 days from start date. Below please find our current employee contributions (per biweekly pay period):

Medical Coverage As a PPO, there are certain providers in the network who are favored by Cigna Open Access Plus — they have worked out specific rates with them and we reap the benefits of the discounts. Telemedicine Virtual care lets you get the care you need — including most prescriptions (when appropriate) — for a wide range of minor conditions. • Who : Board-certified doctors and pediatricians as well as licensed counselors and psychiatrists. • When : Medical conditions: 24/7/365 day or night, including weekends and holidays. Behavioral health: schedule an appointment. • How : Phone or video.
Dental Care Dental visits do not require a network provider. However, employees are encouraged to use a provider from the Cigna Dental PPO Network to benefit from the discounts provided through the network. The plan provides for two free cleanings a year and varying percentages of coverage depending on the work that is being done. Orthodontia is provided for children only. Vision Care Vision visits do not require a network provider. However, employees are encouraged to use a provider from the Cigna Vision Plan to benefit from the discounts provided through the network. This benefit includes: eye exams ($20 co-pay, any provider), frames, lenses, contacts, and prescription sunglasses. There is a $200 maximum coverage per member, per calendar year.

physical exam and includes immunizations, lab tests, screenings and other services intended to prevent illness or detect problems before you notice any symptoms. • Pharmacy coverage : Medications are placed in categories based on drug cost, safety and effectiveness. These tiers also affect your coverage. • GENERIC — A drug that offers equivalent uses, doses, strength, quality and performance as a brand-name drug, but is not trademarked. • BRAND PREFERRED — A drug with a patent and trademark name that is considered “preferred” because it is appropriate to use for medical purposes and is usually less expensive than other brand- name options. • BRAND NON-PREFERRED — a drug with a patent and trademark name. This type of drug is “not preferred” and is usually more expensive than alternative generic and brand preferred drugs. • SPECIALTY — A drug that requires special handling, administration or monitoring. Most can only be filled by a specialty pharmacy and have additional required approvals.
Prescription coverage The health insurance plan includes prescription coverage. There are preferred pharmacies to use, but almost all pharmacies fall under the heading of preferred. Under the prescription drug plan, there will be a “formulary” or list of drugs to reference when obtaining medications. Making the most of your plan Getting the most out of your plan also depends on how well you understand it. Keep these important tips in mind: • In-network providers and pharmacies: You will always pay less if you see a provider within the medical and pharmacy network. • Mail order pharmacy: If you take a maintenance medication on an ongoing basis for a condition like high cholesterol or high blood pressure, you can use the mail order pharmacy to save on a 90- day supply of your medication. • Preventive care: In-network preventive care is covered at 100% (no cost to you). Preventive care is often received during an annual

Benefit Plan Provisions IN-NETWORK REIMBURSEMENT: For services provided by a Cigna Dental PPO network dentist, Cigna Dental will reimburse the dentist according to a Fee Schedule or Discount Schedule. NON-NETWORK REIMBURSEMENT: For services provided by a non- network dentist, Cigna Dental will reimburse according to the Maximum Reimbursable Charge. The MRC is calculated at the 80th percentile of all provider charges in the geographic area. The dentist may balance bill up to their usual fees.
CALENDAR YEAR BENEFITS MAXIMUM: The plan will only pay for covered charges up to the yearly Benefits Maximum, when applicable. Benefit-specific Maximums may also apply. BENEFITS OF USING IN-NETWORK DENTAL PROVIDERS While you have the option of choosing any provider, you will save money when you use in-network dentists. When using an out-of- network dental provider, you will pay more because the provider has not agreed to charge you a negotiated rate.

START IT Contributions to the HSA are tax-free for you whether they come from you or the company. BL Companies contributes $750 for individual coverage and $1,500 for employee + one or more. The contribution will be provided to employees in equal installments per pay period. Plans with an HSA typically cost less than other plans so the money you save on premiums can be put into your HSA. You save money on taxes and have more flexibility and control over your health care dollars. BUILD IT All of the money in your HSA is yours (including any contributions deposited by the company) even if you leave your job, change plans or retire. In 2022, the total of your contributions and the company’s can be up to $3,650 for individual coverage and $7,300 for family coverage. USE IT You can withdraw your money tax-free at any time, as long as you use it for qualified expenses (a list can be found on www.irs.gov). You can also save this money and hold onto it for future eligible health care expenses. GROW IT Unused money in your HSA will roll over, earn interest and grow tax-free over time. You decide how to use the HSA money, including whether to save it or spend it for eligible expenses. When your balance is large enough, you can invest it— tax-free.
HEALTH SAVINGS ACCOUNT (HSA) Available to those enrolled in an HDHP as long as you are not enrolled in any other health coverage or Medicare, or claimed as a dependent on someone else’s tax return. A Health Savings Account (HSA) is a savings account that belongs to you that is paired with an HDHP. It allows you to make tax-free contributions to a savings account to pay for current and future medical expenses for you and your dependents.

ESOP Fundamentals An ESOP is an employee retirement benefit plan through which all company employees have an opportunity to own company stock without having to pay for it. Each year, BL Companies can contribute cash to the ESOP. The ESOP in turn uses that cash to buy shares. Any extra cash accumulates in the ESOP. Once a year, the cash and shares are allocated among all employees who are eligible ESOP participants. The value of the shares is determined each year by an outside valuation firm. When an employee leaves BL Companies, they are entitled to the vested value of their account.
RETIREMENT PLANS
401(k) Plan administered by The Principal Group Employees are eligible to participate in the 401(k) plan immediately after receiving their first paycheck. Part-time employees have to put in 1000 hours of service over a 12-month consecutive period. There are funds from various asset classifications to choose from and an employee can contribute up to the maximum provided by IRS regulation before taxes. 401(k) Match BL Companies provides a 401(k) match. The 401(k) match is 100% on the first 3% and 50% on the next 2%.

LONG TERM AND SHORT TERM DISABILITY BL Companies provides full-time employees with Long & Short Term Disability Insurance from Sun Life Assurance Company — free of charge. This insurance is not for cases where an employee is disabled on the job, that would be considered a workers’ compensation claim. Long Term Disability Long Term Disability pays 60% of an employee’s salary up to a maximum benefit of $10,000 per month to the age of 65. It has a pre-existing condition clause, which states that if the individual has been to the doctor for anything in the 3 months before coverage under the plan, they will not be covered until they have satisfied a 12-month waiting period. Short Term Disability Short Term Disability pays 70% of an employee’s salary up to a maximum benefit of $2,500 per week for up to 12 weeks. There is no pre-existing condition clause, but there is a 7-day “elimination” period (that means one has to be under a doctor’s care for 7 days to qualify for short term disability). An employee can use PTO to supplement the STD benefit to get up to 100% of pre-disability earning.
LIFE INSURANCE All full-time employees receive a $50,000 life insurance policy whether or not they choose to become members of the health insurance plan. It is provided by the company at no cost to the employee. Voluntary Life Insurance BL Companies offers employees the opportunity to enroll and increase Life Insurance coverage in the Voluntary Life Insurance Plan as long as they remain an eligible employee and continue to pay premiums at a group rate.
