The Point System will take into account records found after the Employee’s initial date of hire and will remain under review for 3 consecutive years, with the initial year dropping off. For any violations below, Employees will complete an incident report which shall be placed in their personnel file.

Highest Risk Leaving scene of an accident. Fleeing officer. Any drug and/or alcohol related offense. Refusal to submit to a drug and/or alcohol test. DUI/DWI Any felony, homicide or manslaughter involving use of a motor vehicle. Reckless, negligent driving to endanger. Note: Before taking adverse action in an employment decision based on the Employee’s driving record, the Fair Credit Reporting Act requires the Company to provide the Employee with a copy of the report along with a written description of the Employee’s rights under the Fair Credit Reporting Act. Supervisor to advise Employee that his or her driving record has resulted in revoked driving privilege. If the Employee can no longer perform the essential functions of his or her position or other job assignments are not available, termination could result. 10
