EMPLOYEE ACKNOWLEDGEMENT FORM The BL Companies, Inc. Corporate Safety Manual has been designed to use as a personal reference in answering questions about safety and health as it relates to individual Employees’ jobs and the Company’s safety and health policies as a whole. The Company reserves the right to change this Manual at any time it determines a need to do so. If at any time an Employee is unclear as to any of BL Companies’ safety and health policies, please talk to the Designated Safety Coordinator in your Department or your Department Director. I UNDERSTAND THAT THE INFORMATION CONTAINED IN THIS MANUAL REPRESENTS COMPANY GUIDELINES, WHICH MAY BE MODIFIED FROM TIME TO TIME. I ACKNOWLEDGE THAT I HAVE REVIEWED A COPY OF THE BL COMPANIES, INC.’s CORPORATE SAFETY MANUAL AND UNDERSTAND MY OBLIGATION TO FAMILIARIZE MYSELF WITH ITS CONTENTS AND ABIDE BY THE POLICIES AND PROCEDURES SET FORTH THEREIN.
