Hazard Communications In accordance with 29 CFR 1910.1030, BL Companies will use warning labels affixed to containers of regulated waste, refrigerators and freezers and other containers used to store or transplant blood or OPIM and signs to communicate hazards. BL Companies may also use red biohazard bags or containers instead of labels. BL Companies will post signs to identify restricted areas. Records If necessary, BL Companies will maintain Employee medical and training records. Training records will be maintained for three (3) years from the date of the training and medical records will be maintained for the duration of the employment plus thirty (30) years. BL Companies will also maintain a Sharps Injury Log unless classified as an exempt industry under OSHA’s standard on Recording and Reporting Occupational Injuries and Illnesses. BL Companies will ensure that these records be made available upon request of the Employee. These records must have written consent from the Employee prior to being released. Purpose The purpose of a Pandemic Preparedness Plan is to plan and take appropriate measures to 1) ensure that as few Employees as possible contract the disease, and 2) minimize business interruption as much as possible. Scope This Section applies to all BL Companies’ Employees and all BL operations. Policy BL Companies’ policy is to protect its Employees and ensure continuous operations for its clients and business partners during a pandemic. Overview Business continuity means ensuring that essential business functions can survive a natural disaster, technological failure, human error or other disruption. Many existing business continuity plans anticipate disruptions such as fires, earthquakes and floods, and these events are restricted to certain geographic areas with time frames fairly well defined and limited. A pandemic, however, demands a different set of continuity assumptions since it 7.8 Pandemic Preparedness Plan

will be widely dispersed geographically and potentially arrives in waves that could last several months or years at a time. If a pandemic strikes, BL Companies will issue information on how to avoid becoming ill based on advice from State and local health officials as well as the Centers for Disease Control. Company Directors, the Human Resource Department and Employees should pay close attention to the guidance provided by these officials. · Provide each Employee information to prepare themselves or their families; · Put measures in place to minimize the spread of the pandemic in the workplace; · Monitor Employee absentee rates; · Create a system to notify/share information with Employees during the pandemic; · Invest in the technology to allow Employees to work from home as much as possible; and · Develop a plan to address essential resources to maintain operations. Assumptions Different pandemics will spread in different ways. A pandemic flu would spread rapidly and easily from person-to-person, affecting the business due to absenteeism. Other businesses will be facing the same massive absentee rates and will be unable to provide essential components to maintain the daily operations. Other pandemics may be slower moving and cause more damage due to Employee fear rather than physical sickness. Regardless of the type, BL Companies will perform a risk assessment to identify the essential/critical components of the business’s operation that may need to be operational during the pandemic. BL Companies will at a minimum do the following:
